Chapter -4 

Define extraction- It is define as the pain less removal of tooth or, tooth root with minimal trauma to investing tissue so, that the wound heals eventfully and no post operative problem created. 

( এটা একটা ব্যথা মুক্ত পদ্ধতিতে দাঁত বা দাঁতের রুট কম আঘাতের মাধ্যমে তোলা হয়,এবং পরবর্তীতে ক্ষত শুকানোর সময় কোন সমস্যার সৃষ্টি হবে না।)

Indication of tooth extraction (দাঁত তোলার নিদেশিকা)

1. Gross caries.(অনেক বড় ক্যারিজ হলে) 

2. Pulpitis. (if RCT is not possible) (পাল্পে প্রদাহ)

3. Apical periodontists.( দাঁতের রুটের চুড়ায় প্রদাহ হলে)

4. Extensive loss of alveolar bone. (অতিরিক্ত পরিমান দাঁতের হাড় ক্ষয় হলে)

5. Fracture tooth. (দাঁত ভেঙে গেলে)

6. Fracture jaw. (চোয়াল ভেঙে গেলে)

7. Misplaced or Impacted tooth. (দাঁত সঠিক অবস্থানে না উঠলে)

8. Retained Deciduous tooth.(বাচ্চাদের দাঁত না পড়লে)

9. Supernumerary and supplementary tooth.(অতিরিক্ত দাঁত ক্ষুদ্র আকারে উঠলে)

10. For orthodontic treatment purpose.(আকা বাক দাঁতের চিকিৎসার ক্ষেত্রে)

11. Treatment of cyst and tumor.(সিস্ট বা টিউমার চিকিৎসার সময়)

12. Astetic consideration.(সৌন্দর্য়্য বৃদ্ধির চিকিৎসায়)

Contra-Indication of Extraction (দাঁত তোলার বিধি নিষেধ)

1. Acute gingival infection.(হঠাৎ মাড়িতে প্রদাহ হলে)

2. Acute perichoronitis.( হঠাৎ পেরিকরোনইটিস হলে)

3.Acute Maxillary sinusitis.( হঠাৎ ম্যাক্সিলারি সাইনাসে প্রদাহ হলে)

4. Acute cellulitis. ( হঠাৎ সেলুলাইটিস হলে)

5. Acute infectious osteomailitis.( হঠাৎ হাড়ের প্রদাহ হলে)

6. Oral malignancy.( মুখের ক্যান্সার হলে)

7. Hypertension ( when uncontroll) (অনিয়ন্ত্রিত উচ্চ রক্তচাপ 

8. Reumatic heart diseases.(বাতজ্বর জনিত হৃদরোগ)

9. Uncontrolled Diabetic mallitus.( অনিয়ন্ত্রিত ডায়বেটিক)

10. Ischemic heart diseases.( বুকে ব্যাথা )

11. Myocardial infraction. (হৃদপিন্ডের কপাটিকায় জীবানু সংক্রামন)

12. Sever anemia, leukeria, ITP, Haemorrhagic purpura.(মারাত্বক রক্তস্বল্পতা, রক্তক্ষরন জনিত সমস্যা)

13. Chronic renal failure.(কিডনি ফেইল করলে)

14. Jaundice.(জন্ডিস)

15.  High fever.(অত্যাধিক জ্বর)

16. Pyrexia or fever of unknown origin.(অজানা কারনে জ্বর, খিচুনি)

Complication of tooth extraction (দাঁত তোলার জটিলতা)

1. Immediate complication 

a) Syncope b) Shoke. c) Excessive bleeding. d) Fracture of tooth. e) Injury to the adjacent tooth.

f) Injury to the gum. g) Injury to the jaw. h) Breakage of needle. i) Incomplete extraction.

j) Nerve damage.  k) Accidental open maxillary sinus. l) Bleeding per socket.

2. Delayed complication

a) Dry socket. b) Osteomylitis. c) Infective endocarditis. d) Fever. e) Swilling. f) Redness. 

g) Retained root. h) Granuloma. i) Malignancy. j) Cyst formation. 

Advice After Extraction

1. Avoid biting lips, cheek or tongue during this time.

2. No hot and hard food for 24 hours. 

3. Only cold and soft food should be taken. 

4. Bite on the cotton pad with fine constsnt pressure.

5. Some blood may continue to oozo into the mouth over the next few hours. so do not use cotton wool or tissue paper.

6. Avoid vigorous rinsing for 24 hours.

7. Avoid alcohol drinking 24 hours.

8. Warm saline gurgling after 24 hours of extraction. 

9. Come after 2 days for follow up and after 7 days for stich removal.
